Withania somnifera

Ashwagandha root extracts may reduce stress and anxiety and improve sleep. A specific extract may reduce fatigue in adults with existing complaints; studies have also observed improvements in some measures of mood, memory and attention. With strength training, ashwagandha may increase strength, and in physically active people it may improve measures of aerobic performance. In professional female footballers, one extract improved subjective ratings of recovery. In adults with overweight, KSM-66 may reduce body weight. In women with menopausal or sexual complaints, certain extracts may improve the corresponding subjective measures. In men with sexual complaints, KSM-66 may improve sexual desire and some sperm measures. In one study, KSM-66 improved some measures of skin and hair. Prevention of infections has not been shown.

Population studiedAdults with stress

Attention, memory and cognitive measures.

Confidence1/5

Improvements in individual cognitive tests are possible. A noticeable improvement in everyday memory and attention has not been established.

For memory and attention, look at specific tests rather than generalizing the result to all cognitive functions.

Study result. In the review, the difference for memory tests was SMD 0.52 (95% CI 0.27–0.78), and for attention and speed 0.29 (0.07–0.51). These are standardized estimates across different tests, not percentages.

Studied regimenIn a separate RCT in 130 adults with stress, extended-release root extract 300 mg/day was studied for 90 days; placebo
Population studiedAdults in studies of individual cognitive tests

Studied regimenKSM-66, root extract 300 mg twice daily for 12 weeks; placebo
Population studiedPhysically active adults

Recovery and soreness after training.

Confidence1/5

Consistently faster recovery and reduced post-exercise muscle soreness have not been established.

For recovery, there is an individual result in professional female footballers.

Study result. In one RCT, subjective recovery ratings improved in professional female footballers. In other groups, recovery and soreness results were mixed.

Studied regimenRoot extract 600 mg/day for 28 days; placebo
Population studied30 professional female footballers in one study

Body weight and composition.

Confidence2/5

One study of a specific extract reported weight loss in people with overweight.

KSM-66 was studied for body weight in adults with overweight.

Study result. In one RCT, body weight changed by −8.46 versus −2.41 kg. Both groups were instructed to maintain their usual diet and activity.

Studied regimenKSM-66, root extract 600 mg/day for 24 weeks; placebo
Population studied100 adults with stress and BMI 25–39.9; 91 completed the study

Male fertility and sperm parameters.

Confidence1/5

May improve sexual desire and some sperm measures in the studied groups. An increased likelihood of conception and live birth has not been established.

For sperm measures, consider the specific measure assessed.

Study result. The difference in change in total sperm count per ejaculate was +21.20 million (95% CI 6.09–36.30); there was no convincing difference in concentration or motility. Conception and live birth were not assessed.

Studied regimenKSM-66, root extract 300 mg twice daily for 8 weeks; placebo
Population studiedMen with sexual complaints

Testosterone and sexual function.

Confidence2/5

Testosterone and sexual desire changed in individual studies. A predictable increase in testosterone in a healthy consumer has not been established.

Subjective sexual desire was studied in people with sexual complaints.

Safety and limitations

May alter thyroid measures; cases of thyrotoxicosis have been described. People with thyroid disease should avoid taking it on their own.

Drowsiness, nausea, diarrhea and abdominal discomfort may occur. Rare serious liver injury has been reported. People with existing liver disease should avoid use. Jaundice, dark urine and severe itching require stopping use and urgent medical assessment.

Drowsiness, nausea, diarrhea and abdominal discomfort may occur. Rare serious liver injury has been reported. People with existing liver disease should avoid use. Jaundice, dark urine and severe itching require stopping use and urgent medical assessment.

Interactions are possible with sedatives, anticonvulsants, blood glucose-lowering, blood pressure-lowering, immunosuppressive and thyroid medicines. Check compatibility with the specific medicines.

Avoid use during pregnancy and breastfeeding. Studies in children do not justify applying an adult regimen to a child.
