Fatal and non-fatal liver disorders of an idiosyncratic or hypersensitivity type may occur with Ryanodex Dantrolene Sodium therapy.
Patients should be instructed to contact their physician should signs or symptoms of hepatotoxicity (e.g., discoloured faeces, generalised pruritus, jaundice, anorexia, nausea, vomiting) occur during therapy.
Factors that may increase the risk of developing hepatotoxicity include:
- Higher daily doses (doses exceeding 400 mg daily)
- Duration of therapy (most frequently reported between 2 and 12 months of treatment)
- Female gender
- Age greater than 30 years
- Prior history of liver disease/dysfunction
- Receiving other hepatotoxic therapies concomitantly.
Spontaneous reports also suggest a higher proportion of hepatic events with fatal outcome in elderly patients.
At the start of Ryanodex Dantrolene Sodium therapy, it is desirable to do liver function studies (SGOT/AST, SGPT/ALT, alkaline phosphatase, total bilirubin) for a baseline or to establish whether there is pre-existing liver disease. If baseline liver abnormalities exist and are confirmed, there is a clear possibility that the potential for Ryanodex Dantrolene Sodium hepatotoxicity could be enhanced, although such a possibility has not yet been established.
Liver functions studies (e.g. serum, SGOT/AST, SGPT/ALT) should be performed at appropriate intervals during Ryanodex Dantrolene Sodium therapy. If such studies reveal abnormal values, therapy should generally be discontinued. Only where benefits of the drug have been of major importance to the patient, should re-introduction or continuation of therapy be considered. Some patients have revealed a return to normal laboratory values in the face of continued therapy while others have not.
If symptoms compatible with hepatitis, accompanied by abnormalities in liver function tests or jaundice appear, Ryanodex Dantrolene Sodium should be discontinued. If caused by Ryanodex Dantrolene Sodium and detected early, the abnormalities in liver function have reverted to normal when the drug was discontinued.
Ryanodex Dantrolene Sodium has been re-introduced in a few patients who have developed clinical signs, or elevated serum enzymes, of hepatocellular injury.
Re-introduction of Ryanodex Dantrolene Sodium therapy should only be contemplated in patients who clearly need the drug, and only after complete reversal of the signs of hepatotoxicity and liver function tests. Patients being re-challenged with Ryanodex Dantrolene Sodium should be hospital in-patients, and small, gradually increasing doses should be used. Laboratory test monitoring should be frequent, and the drug should be withdrawn immediately if there is any indication of recurrent liver abnormality. Some patients have reacted with unmistakable signs of liver abnormality upon administration of a challenge dose, whilst others have not.
The use of Ryanodex Dantrolene Sodium with other potentially hepatotoxic drugs should be avoided.
There are isolated cases of possibly significant effects of Ryanodex Dantrolene Sodium on the cardiovascular and respiratory systems. These cases also have other features suggesting a pre-disposition to cardiovascular disease, and impaired respiratory function, particularly obstructive pulmonary disease. Ryanodex Dantrolene Sodium should be used with caution in such patients.
Caution should be exercised in the simultaneous administration of tranquillising agents and alcohol.
Absorção
O dantroleno sódico é fácil e quase completamente absorvido pelo trato gastrointestinal. Após a administração com o estômago vazio, os níveis plasmáticos de dantroleno sódico atingem um pico dentro de três horas na maioria dos indivíduos.
Distribuição
O dantroleno sódico é um medicamento altamente lipofóbico. Além disso, falta hidrofilicidade. O dantroleno sódico se liga à albumina sérica humana (HSA) com uma razão molar de 0,95 a 1,68 in vitro. A constante de associação in vitro é maior (2,3 a 5,4 x 10 -5 por mol). O antroleno sódico in vitro pode ser deslocado do HSA por varfarina, clofibrato e tolbutamida, mas essas interações não foram confirmadas em humanos (re. banco de dados do fabricante). Estudos de dose intravenosa única sugerem que o volume primário de distribuição é de cerca de 15 litros. As doses orais únicas atingem um pico de concentração plasmática de cerca de um quarto da concentração para uma dose intravenosa de tamanho semelhante.
Metabolismo e Eliminação
A meia-vida biológica no plasma na maioria dos seres humanos é entre 5 e 9 horas, embora meias-vidas até 12,1 ± 1,9 horas tenham sido relatadas após uma única dose intravenosa. A inativação é pelo metabolismo hepático em primeira instância. Existem duas vias alternativas. A maior parte do medicamento é hidroxilada a 5-hidroxi-dantroleno. A via menor envolve redução de nitro para amino-dantroleno, que é então acetilado (composto F-490). O metabolito 5-hidroxi é um relaxante muscular com quase a mesma potência que a molécula parental e pode ter uma meia-vida mais longa que o composto original. O composto F-490 é muito menos potente e provavelmente está inativo nas concentrações alcançadas em amostras clínicas. Os metabólitos são subsequentemente excretados na urina na proporção de 79 5-hidroxi-dantroleno: 17 composto F-490: 4 dantroleno inalterado (sal ou ácido livre). A proporção de fármaco excretado nas fezes depende do tamanho da dose.
