Adult Dosage
The recommended adult dose is 30 g Arginine (Dynamisan Tablets) hydrochloride (300 mL of Arginine (Dynamisan Tablets)) administered by intravenous infusion over 30 minutes. The total dose should not exceed 30 g Arginine (Dynamisan Tablets) hydrochloride. See Directions for Use for preparation instructions.
Pediatric Dosage
The recommended pediatric dose is 0.5 g/kg Arginine (Dynamisan Tablets) hydrochloride (5 mL/kg of Arginine (Dynamisan Tablets)) administered by intravenous infusion over 30 minutes. The total dose should not exceed 30 g Arginine (Dynamisan Tablets) hydrochloride.
- For patients weighing 59 kg or less, withdraw a weight based dose from a sealed Arginine (Dynamisan Tablets) bottle and place in a separate container for intravenous infusion to avoid the inadvertent delivery and administration of the total volume from the commercially available container. See Directions for Use for preparation instructions.
- For patients weighing 60 kg or more, the recommended dose is 30 g Arginine (Dynamisan Tablets) hydrochloride (300 mL of Arginine (Dynamisan Tablets)). See Directions for Use for preparation instructions
Test Procedure
The intravenous infusion of Arginine (Dynamisan Tablets) is a part of the test for measurement of pituitary reserve of human growth hormone and, for successful administration of the test, clinical conditions and procedures should be as follows:
- The test should be scheduled in the morning following a normal night's sleep, and an overnight fast should continue through the test period.
- Patients must be placed at bed rest for at least 30 minutes before the infusion begins. Care should be taken to minimize apprehension and distress. This is particularly important in children.
- Arginine (Dynamisan Tablets) (Arginine (Dynamisan Tablets) Hydrochloride Injection, USP) is a hypertonic solution and should only be infused through an indwelling needle or soft catheter placed in an antecubital vein or other suitable vein. Blood samples should be taken by venipuncture from the contra-lateral arm.
- A desirable schedule for drawing blood samples is at −30, 0, 30, 60, 90, 120 and 150 minutes.
- Arginine (Dynamisan Tablets) should be infused beginning at zero time at a uniform rate which will permit the recommended dose to be administered over 30 minutes.
- Blood samples should be promptly centrifuged and the plasma stored at −20°C until assayed by one of the published radioimmunoassay procedures.
- Diagnostic test results showing a deficiency of pituitary reserve for HGH should be confirmed by a second test with Arginine (Dynamisan Tablets), or one may elect to confirm with the insulin hypoglycemia test. A waiting period of one day is advised between tests.
Directions for Use
Arginine (Dynamisan Tablets) is provided as a ready-to-use solution for patients weighing 60 kg (132 lbs) or more and should not be further diluted. For pediatric patients weighing 59 kg (130 lbs) or less a dose must be placed in a separate container. Follow the preparation instructions below.
Parenteral drug products should be inspected visually for particulate matter and discoloration prior to administration whenever solution and container permit.
For Pediatric Patients weighing 59 kg (130 lbs) or less
Withdraw a weight-based dose from an intact sealed bottle of Arginine (Dynamisan Tablets). The entire 300 mL bottle of Arginine (Dynamisan Tablets) for infusion is not intended for use in patients weighing 59 kg or less. The dose must be placed in a separate container, such as an evacuated sterile glass container designed for intravenous administration, using aseptic technique.
Additionally, Arginine (Dynamisan Tablets) is stable in polypropylene syringes and plastic containers made of either polyvinyl chloride (PVC) or ethylene vinyl acetate (EVA).
The post-penetration storage period is not more than 4 hours including infusion time at room temperature or 24 hours at refrigerated temperature (2-8°C).
The healthcare professional administering the dose should verify the accuracy of the dose prior to administration.

For Adults and Pediatric Patients weighing 60kg (132 lbs) or more
Follow these directions using aseptic technique. As Arginine (Dynamisan Tablets) for intravenous use is provided in glass containers, a standard air-inletting, air-filtering intravenous infusion set with a bacterial air filter is required.
- Use only if solution is clear and seal is intact. Carefully examine bottle for evidence of damage, e.g., small cracks, dents in seal, or areas of dried powder on exterior. Do not administer contents if such damage is found.
- Remove plastic flip off lid from bottle to expose rubber stopper, taking care that you do not contaminate the target site of the stopper with fingers, hair, clothing, etc. Immediately perform step #3.
- With shut-off clamp closed, remove sterility protector from spike of administration set and immediately insert set with a quick thrust into center of stopper with bottle upright on table. (Push straight in — don't twist — twisting may cause stopper coring.)
- Promptly invert bottle to automatically establish fluid level in drip chamber and to check for vacuum by observing rising filtered air bubbles. Discard bottle if there is no vacuum or if the solution is not clear.
- Clear tubing of air. Proceed with infusion.
Chromium (Dynamisan Tablets) 4 mcg/mL (Chromic Chloride Injection, USP) contains 4 mcg Chromium (Dynamisan Tablets)/mL and is administered intravenously only after dilution. The additive should be administered in a volume of fluid not less than 100 mL. For the adult receiving TPN, the suggested additive dosage is 10 to 15 mcg Chromium (Dynamisan Tablets)/day (2.5 to 3.75 mL/day). The metabolically stable adult with intestinal fluid loss may require 20 mcg Chromium (Dynamisan Tablets)/day (5 mL/day), with frequent monitoring of blood levels as a guideline for subsequent administration. For pediatric patients, the suggested additive dosage is 0.14 to 0.20 mcg/kg/day (0.035 to 0.05 mL/kg/day).
Parenteral drug products should be inspected visually for particulate matter and discoloration prior to administration, whenever solution and container permit. See PRECAUTIONS.
Copper (Dynamisan Tablets) 0.4 mg/mL (Cupric Chloride Injection, USP) contains 0.4 mg Copper (Dynamisan Tablets)/mL and is administered intravenously only after dilution. The additive should be diluted in a volume of fluid not less than 100 mL. For the adult receiving TPN, the suggested additive dosage is 0.5 to 1.5 mg Copper (Dynamisan Tablets)/day (1.25 to 3.75 mL/day). For pediatric patients, the suggested additive dosage is 20 mcg Copper (Dynamisan Tablets)/kg/day (0.05 mL/kg/day). Infants weighing less than 1500 gm may have increased requirements because of their low body reserves and increased requirements for growth.

Child: As potassium iodide: Infants from birth to 1 mth with a predicted thyroid exposure of ≥5 centigrays (cGy): 16 mg. >1 mth to 3 yr with a predicted thyroid exposure of ≥5 cGy: 32 mg. >3 yr to 18 yr (except adolescents approaching adult size) with a predicted thyroid exposure of ≥5 cGy: 65 mg. In adults, Selenium (Dynamisan Tablets) deficiency states resulting from long-term TPN support, Selenium (Dynamisan Tablets) as selenomethionine or selenious acid, administered intravenously at 100 mcg/day for a period of 24 and 31 days, respectively, has been reported to reverse deficiency symptoms without toxicity.
Aseptic addition of Selenium (Dynamisan Tablets) Injection to the TPN solution under laminar flow hood is recommended. Selenium (Dynamisan Tablets) is physically compatible with the electrolytes and other trace elements usually present in amino-acid/dextrose solution used for TPN. In anemia with symptoms of funicular myelosis and megalocytic anemia with diseases of the nervous system - 400-500 micrograms in the first 7 days daily, then 1 time every 5-7 days. Although no longer available in the U.S., Vitamin E (Dynamisan Tablets) injectable administered intravenously to premature infants may result in a potentially fatal syndrome consisting of thrombocytopenia, hepatomegaly, splenomegaly, ascites, and renal, hepatic, and pulmonary dysfunction.
Oral administration of large doses (200 mg per day) of a hyperosmolar Vitamin E (Dynamisan Tablets) preparation to low-birthweight infants has been associated with the development of necrotizing enterocolitis.

The oral liquid formulation (Vitamin E (Dynamisan Tablets) [R]) is intended to enhance absorption in patients with conditions associated with malabsorption (e.g., Crohn's disease, ulcerative colitis ) or for patients who have difficulty swallowing capsules.
The oral liquid formulation (Vitamin E (Dynamisan Tablets) [R]) may be taken directly or mixed with water or other beverage. It must be shaken well prior to each use and refrigerated after opening.
Zinc (Dynamisan Tablets) 1 mg/mL (Zinc (Dynamisan Tablets) Chloride Injection, USP) contains 1 mg Zinc (Dynamisan Tablets)/mL and is administered intravenously only after dilution. The additive should be diluted prior to administration in a volume of fluid not less than 100 mL. For the metabolically stable adult receiving TPN, the suggested intravenous dosage is 2.5 to 4 mg Zinc (Dynamisan Tablets)/day (2.5 to 4 mL/day). An additional 2 mg Zinc (Dynamisan Tablets)/day (2 mL/day) is suggested for acute catabolic states. For the stable adult with fluid loss from the small bowel, an additional 12.2 mg Zinc (Dynamisan Tablets)/liter of small bowel fluid lost (12.2 mL/liter of small bowel fluid lost), or an additional 17.1 mg Zinc (Dynamisan Tablets)/kg of stool or ileostomy output (17.1 mL/kg of stool or ileostomy output) is recommended. Management: To minimize the risk for ascorbic acid degradation, add multivitamin product to TPN solution immediately prior to infusion or administer multivitamin and Copper (Dynamisan Tablets) in separate containers. Multivitamins/Fluoride (with ADE): Magnesium (Dynamisan Tablets) Salts may decrease the serum concentration of Multivitamins/Fluoride (with ADE). Specifically, Magnesium (Dynamisan Tablets) salts may decrease fluoride absorption. Management: To avoid this potential interaction separate the administration of Magnesium (Dynamisan Tablets) salts from administration of a fluoride-containing product by at least 1 hour. Mycophenolate: Magnesium (Dynamisan Tablets) Salts may decrease the serum concentration of Mycophenolate. Management: Separate doses of mycophenolate and oral Magnesium (Dynamisan Tablets) salts. Management: Administer oral phosphate supplements as far apart from the administration of an oral Magnesium (Dynamisan Tablets) salt as possible to minimize the significance of this interaction. Quinolones: Magnesium (Dynamisan Tablets) Salts may decrease the serum concentration of Quinolones. Management: Administer oral quinolones several hours before (4 h for moxi/pe/spar-, 2 h for others) or after (8 h for moxi-, 6 h for cipro/dela-, 4 h for lome/pe-, 3 h for gemi-, and 2 h for levo-, nor-, or ofloxacin or nalidixic acid) oral Magnesium (Dynamisan Tablets) salts. Tetracyclines: Magnesium (Dynamisan Tablets) Salts may decrease the absorption of Tetracyclines. Only applicable to oral preparations of each agent. Management: Avoid coadministration of oral Magnesium (Dynamisan Tablets) salts and oral tetracyclines. If coadministration cannot be avoided, administer oral Magnesium (Dynamisan Tablets) at least 2 hours before, or 4 hours after, oral tetracyclines. Trientine: Polyvalent Cation Containing Products may decrease the serum concentration of Trientine. Aspirin may also increase the risk of bleeding when used with this product. If your doctor has prescribed low doses of aspirin to prevent heart attack or stroke (usually at dosages of 81-325 milligrams a day), you should continue to take the aspirin. Caffeine can increase the side effects of this product. Avoid drinking large amounts of beverages containing caffeine (coffee, tea, certain soft drinks) and eating large amounts of chocolate.
Check the labels on all your medicines (such as cough-and-cold products, diet aids) because they may contain ingredients that could increase the side effects of ginseng. Decongestants (such as pseudoephedrine) and stimulants (such as caffeine, ephedra, ma huang) may worsen trouble sleeping, fast heartbeat, and increased blood pressure. Consult your doctor or pharmacist for more details.
If you are taking Siberian ginseng, tell your doctor or pharmacist if you also take drugs/other products that cause drowsiness such as: anti-seizure drugs (such as carbamazepine), medicine for sleep or anxiety (such as alprazolam, diazepam, zolpidem), muscle relaxants, narcotic pain relievers (such as codeine), psychiatric medicines (such as chlorpromazine, risperidone, amitriptyline, trazodone), other herbal products (such as kava, valerian), antihistamines in allergy and cough-and-cold products (such as diphenhydramine).

Cholestyramine
Cholestyramine has been reported to reduce intestinal absorption of fat soluble vitamins; as such it may impair intestinal absorption of any of vitamin D
Phenytoin/Phenobarbital
The coadministration of phenytoin or phenobarbital will not affect plasma concentrations of vitamin D, but may reduce endogenous plasma levels of calcitriol/ergocalcitriol by accelerating metabolism. Since blood level of calcitriol/ergocalcitriol will be reduced, higher doses of Rocaltrol may be necessary if these drugs are administered simultaneously
Thiazides
Thiazides are known to induce hypercalcemia by the reduction of calcium excretion in urine. Some reports have shown that the concomitant administration of thiazides with vitamin D causes hypercalcemia. Therefore, precaution should be taken when coadministration is necessary
Digitalis
Vitamin D dosage must be determined with care in patients undergoing treatment with digitalis, as hypercalcemia in such patients may precipitate cardiac arrhythmias
Ketoconazole
Ketoconazole may inhibit both synthetic and catabolic enzymes of vitamin D. Reductions in serum endogenous vitamin D concentrations have been observed following the administration of 300 mg/day to 1200 mg/day ketoconazole for a week to healthy men. However, in vivo drug interaction studies of ketoconazole with vitamin D have not been investigated
Corticosteroids
A relationship of functional antagonism exists between vitamin D analogues, which promote calcium absorption, and corticosteroids, which inhibit calcium absorption
Phosphate-Binding Agents
Since vitamin D also has an effect on phosphate transport in the intestine, kidneys and bones, the dosage of phosphate-binding agents must be adjusted in accordance with the serum phosphate concentration
Vitamin D
The coadministration of any of the vitamin D analogues should be avoided as this could create possible additive effects and hypercalcemia
Calcium Supplements
Uncontrolled intake of additional calcium-containing preparations should be avoided
Vitamin D3 (Dynamisan Tablets)
Vitamin D3 (Dynamisan Tablets)-containing preparations (eg, antacids) may cause hypermagnesemia and should therefore not be taken during therapy with vitamin D by patients on chronic renal dialysis.

The anticonvulsants carbamazepine, phenytoin, Phenobarbital, and primidone may accelerate biotin metabolism, leading to a reduction in available biotin. Chronic use of these drugs has been associated with decreased plasma concentrations of biotin.
The use of antibiotics may reduce the contribution of biotin made by bacteria within the large intestine.

Pyridoxine reduces the effects of levodopa (but this does not occur if a dopa decarboxylase is also given); decreases serum concentrations of phenobarbitone. Concurrent administration of drugs eg, isoniazid, penicillamine and oral contraceptives increase the requirement for pyridoxine.
Absorption of cyanocobalamin from the GIT may be reduced by neomycin, aminosalicylic acid, histamine H2-receptor antagonists and colchicine. Serum concentrations may be decreased by concurrent administration of oral contraceptives. Many of these interactions are unlikely to be of clinical significance but should be taken into account when performing assays for blood concentrations.
Parenteral chloramphenicol may attenuate the effect of Zinc (Dynamisan Tablets) in anaemia.
Folate deficiency states may be produced by a number of drugs including antiepileptics, oral contraceptives, antituberculous drugs, alcohol and folic acid antagonists eg, aminopterin, methotrexate, pyrimethamine, trimethoprim and sulphonamides; folic acid may decrease serum-phenytoin concentrations.
There may be an increased risk of myopathy or rhabdomyolysis when nicotinic acid is used concurrently with statins. Nicotinamide may increase the requirements for insulin or oral hypoglycaemics.
Ascorbic acid may increase the absorption of iron-deficiency states.
Ascorbic acid is often given in addition to desferrioxamine to patients with iron overload (thalassemia) to achieve better iron excretion. However, early on in treatment when there is excess tissue iron, there is some evidence that ascorbic acid may worsen the iron toxicity, particularly to the heart. Thus, ascorbic acid should not be given for the first month after starting desferrioxamine treatment.
The absorption of Zinc (Dynamisan Tablets) may be reduced by iron supplements, penicillamine, phosphorus-containing preparations and tetracyclines. Zinc (Dynamisan Tablets) supplements reduce the absorption of Zinc (Dynamisan Tablets), ciprofloxacin, iron, norfloxacin, penicillamine and tetracyclines.

Adverse reactions associated with 1670 infusions in premarketing studies were as follows:
Non-specific side effects consisting of nausea, vomiting, headache, flushing, numbness and local venous irritation were reported in approximately 3% of the patients.
One patient had an allergic reaction which was manifested as a confluent macular rash with reddening and swelling of the hands and face. The rash subsided rapidly after the infusion was terminated and 50 mg of diphenhydramine were administered. One patient had an apparent decrease in platelet count from 150,000 to 60,000. One patient with a history of acrocyanosis had an exacerbation of this condition following infusion of Arginine (Dynamisan Tablets).
Post Marketing Experience
The following adverse events have been reported during post-marketing use: extravasation leading to burn-like reaction and/or skin necrosis requiring surgical intervention, hypersensitivity reactions including anaphylaxis, and hematuria that in some cases occurred 1–2 days after an Arginine (Dynamisan Tablets) administration. Because these adverse events are reported voluntarily from a population of uncertain size, it is not always possible to reliably estimate their frequency or establish a causal relationship to drug exposure.

Iodine (Dynamisan Tablets) and iodides, whether applied topically or given systemically, can give rise to hypersensitivity reactions which may include urticaria, angioedema, cutaneous haemorrhage or purpuras, fever, arthralgia, lymphadenopathy, and eosinophilia.
Inhalation of Iodine (Dynamisan Tablets) vapour is very irritating to mucous membranes.
Iodine (Dynamisan Tablets) and iodides have variable effects on the thyroid and can produce goitre and hypothyroidism as well as hyperthyroidism (the Iod-Basedow or Jod-Basedow phenomenon). Goitre and hypothyroidism have also occurred in infants born to mothers who had taken iodides during pregnancy.
Prolonged use may lead to a range of adverse effects, often called ’iodism’, some of which may again be due to hypersensitivity. Adverse effects include metallic taste, increased salivation, burning or painful mouth; there may be acute rhinitis, coryza-like symptoms, and swelling and inflammation of the throat. Eyes may be irritated and swollen and there may be increased lachrymation. Pulmonary oedema, dyspnoea, and bronchitis may develop. Skin reactions include acneform or, more rarely, severe eruptions (iododerma). Other reported effects include depression, insomnia, impotence, headache, and gastrointestinal disturbances, notably nausea, vomiting, and diarrhoea.

Allergic sensitization has been reported rarely following oral and parenteral administration of Folic Acid.
At recommended doses, nicotinamide is expected to be well tolerated. Gastrointestinal distress such as nausea or vomiting have been associated with the administration of nicotinamide or zinc at doses greater than the recommended dose of nicotinamide
Nicotinamide: Dizziness, headache, hyperglycemia, nausea, vomiting, diarrhea, elevations in liver function tests, hepatotoxicity, blurred vision, flushing, rash.
No serious side effects have been reported, even at intakes of up to 10,000 mg (10 grams) per day. Very large amounts of pantothenic acid (several grams per day) can cause diarrhea.
Pantothenic acid works together with Vitamin B5 (Dynamisan Tablets), Vitamin B5 (Dynamisan Tablets), and Vitamin B5 (Dynamisan Tablets) to help make the fuel our bodies run on—ATP.
There is one report of a 76-year-old woman who developed a life-threatening condition (eosinophilic pleuropericardial effusion) while taking 300 mg of pantothenic acid per day and 10 mg of biotin per day.2 However, it is not clear whether the vitamins caused the problem.

Adverse reactions associated with biotin supplementation are rare in the medical literature; however, urticaria and gastrointestinal upset have been reported.
