Aceril should be used with caution in patients with:

Heart disease (e.g., coronary artery disease, heart failure).

Diabetes mellitus.

Autoimmune diseases (e.g., systemic lupus erythematosus, scleroderma).

Advanced age.
Additional Precautions:
-
A sharp decrease in blood pressure may occur at the beginning of treatment, especially in patients with sodium or fluid deficiency.
-
During therapy it is necessary to regularly monitor renal function, electrolyte levels (potassium, sodium) and general blood analysis.
-
Dry cough is possible.
-
In rare cases angioedema may occur - in case of its occurrence, consult a doctor immediately.
-
Use with caution in patients on hemodialysis.
-
Before surgical intervention it is necessary to inform the doctor about taking captopril.
-
ACE inhibitors may be less effective in patients of negroid race.
-
Use in patients with aortic or mitral stenosis requires special caution.
It is not recommended to take together with:

Potassium-saving diuretics (spironolactone, amiloride, triamterene).
-
Non-steroidal anti-inflammatory drugs (NSAIDs).
-
Allopurinol.
-
Procainamide.
-
Immunosuppressants (azathioprine, cyclophosphamide).

Lithium preparations.
-
mTOR inhibitors (sirolimus, everolimus, temsirolimus).

Insulin and oral hypoglycemic drugs.

Pregnancy: contraindicated. Captopril may cause teratogenic effects and serious fetal abnormalities. In pregnancy, it is necessary to discontinue use and switch to alternative therapy.

Breast-feeding: contraindicated, as captopril penetrates into breast milk.

Fertility: no data on the effect on fertility.
- At the beginning of treatment captopril may cause dizziness, drowsiness, visual disturbances. In the future, the degree of restrictions is determined individually.

Very frequent (≥ 1/10): dizziness, dry cough.

Frequently (≥ 1/100 to < 1/10): taste disturbance, rash, pruritus, orthostatic hypotension, chest pain, fatigue.

Infrequently (≥ 1/1,000 to < 1/100): tachycardia, angioedema, vomiting, constipation, loss of appetite, sleep disturbance.

Rarely (≥ 1/10,000 to < 1/1,000): neutropenia, agranulocytosis, pancytopenia, renal failure, proteinuria, impotence, gynecomastia.

Very rare: Stevens-Johnson syndrome, toxic epidermal necrolysis, photosensitivity.

Symptoms: marked decrease in blood pressure, dizziness, syncope, tachycardia, bradycardia, renal dysfunction, electrolyte imbalance.

Treatment: intravenous administration of saline solution, vasopressors, atropine for bradycardia, adrenaline for angioedema.
