Cysto-Myacyne N
Néomycine
Suppression des bactéries intestinales: Les comprimés de cysto-Myacyne n sulfate sont indiqués comme traitement d'apport en tant que partie d'un régime pour la suppression de la flore bactérienne normale de l'intestin, par exemple, la préparation préopératoire de l'intestin. Il est administré en concomitance avec une base enduite d'érythromycine intestinale (voir POSOLOGIE ET ADMINISTRATION
section).
Coma hépatique (encéphalopathie portale-systématique): Le sulfate de Cysto-Myacyne N s'est avéré être un traitement adjuvant efficace dans le coma hépatique par réduction des bactéries ammoniacales dans le tractus intestinal. La réduction subséquente de l'ammoniac sanguin a entraîné une amélioration neurologique.

Les préparations orales Cysto-Myacyne N sont contre-indiquées en présence d'obstruction intestinale et chez les personnes ayant des antécédents d'hypersensibilité au médicament.
Les Patients ayant des antécédents d'hypersensibilité ou de réaction toxique grave à d'autres aminoglycosides peuvent présenter une sensibilité croisée à la néomycine.
Les préparations orales Cysto-Myacyne N sont contre-indiquées chez les patients atteints d'une maladie gastro-intestinale inflammatoire ou ulcéreuse en raison du potentiel d'absorption gastro-intestinale accumulé de la néomycine.
AVERTISSEMENT
(Voir AVERTISSEMENTS EN BOÎTE).
D'autres manifestations de neurotoxicité peuvent inclure un engourdissement, des picotements cutanés, des contractions musculaires et des convulsions.
Le risque de perte auditive persiste après le retrait du médicament.
Les Aminosides peuvent causer des dommages au fœtus lorsqu'ils sont administrés à une femme enceinte. Les antibiotiques aminoglycosides traversent le placenta et il y a eu plusieurs rapports de surdité congénitale bilatérale totale irréversible chez les enfants dont les mères ont reçu de la streptomycine pendant la grossesse. Bien que des effets secondaires graves sur le fœtus ou le nouveau-né n'aient pas été rapportés dans le traitement des femmes enceintes par d'autres aminoglycosides, le risque de dommages existe. Aucune étude sur la reproduction animale de Cysto-Myacyne N n'a été réalisée. Si Cysto-Myacyne N est utilisé pendant la grossesse, ou si la patiente tombe enceinte pendant la prise de ce médicament, la patiente doit être informée du danger potentiel pour le fœtus.
PRÉCAUTION
Général
Comme avec d'autres antibiotiques, l'utilisation de Cysto-Myacyne n Par voie orale peut entraîner une prolifération d'organismes non sensibles, en particulier de champignons. Si cela se produit, un traitement approprié doit être institué.
Cysto-Myacyne N est rapidement et presque totalement absorbé par les surfaces du corps (sauf la vessie) après une irrigation locale et lorsqu'il est appliqué par voie topique en association avec des interventions chirurgicales. Des cas de surdité irréversible à début retardé, d'insuffisance rénale et de décès dus à un blocage neuromusculaire (quel que soit L'état de la fonction rénale) ont été rapportés après L'irrigation de petits et grands champs chirurgicaux avec des quantités infimes de Cysto-Myacyne N.
L'allergie croisée entre les aminoglycosides a été démontrée.
Les Aminoglycosides doivent être utilisés avec prudence chez les patients présentant des troubles musculaires tels que la myasthénie grave ou le parkinsonisme, car ces médicaments peuvent aggraver la faiblesse musculaire en raison de leur effet potentiel de type curare sur la jonction neuromusculaire.
De petites quantités de Cysto-Myacyne n administré par voie orale sont absorbées par la muqueuse intestinale intacte.
Il y a eu de nombreux rapports dans la littérature de néphrotoxicité et/ou d'ototoxicité avec l'utilisation orale de Cysto-Myacyne N. Si une insuffisance rénale se développe pendant le traitement oral, il faut envisager de réduire la posologie du médicament ou d'interrompre le traitement.
Une dose orale de Cysto-Myacyne N de 12 grammes par jour produit un syndrome de malabsorption pour une variété de substances, y compris la graisse, l'azote, le cholestérol, le carotène, le glucose, le xylose, le lactose, le sodium, le calcium, la cyanocobalamine et le fer.
Cysto-Myacyne n administré par voie orale augmente l'extraction des acides biliaires fécaux et réduit l'activité de la lactase intestinale.
Les Tests De Laboratoire
Les Patients présentant une insuffisance rénale peuvent développer des taux sanguins toxiques de Cysto-Myacyne N à moins que les doses ne soient correctement régulées. Si une insuffisance rénale se développe pendant le traitement, la posologie doit être réduite ou l'antibiotique arrêté. Pour éviter la néphrotoxicité et les lésions du huitième Nerf associées à des doses élevées et à un traitement prolongé, le suivi doit être effectué avant et périodiquement pendant le traitement: analyse d'urine pour une excrétion accumulée de protéines, diminution de la densité, des moulages et des cellules, tests de la fonction rénale tels que la créatinine sérique, la clarté du chignon ou de la la fonction du nerf vestibulocochléaire (huitième nerf crânien).
Des tests sérieux, vestibulaires et audiométriques doivent être effectués (en particulier chez les patients à haut risque). Étant donné que les patients âgés peuvent avoir une fonction rénale réduite qui peut ne pas être évidente dans les résultats des tests de dépistage de routine tels que le chignon ou la créatinine sérique, une détermination de la clarté de la créatinine peut être plus utile.
Carcinogenèse, mutagenèse, altération de la fertilité
Aucune étude à long terme sur l'animal n'a été réalisée avec le N sulfate de Cysto-Myacyne pour évaluer le potentiel cancérogène ou mutagène ou l'altération de la fertilité.

Les Mères Qui Allaient
On ne sait pas si Cysto-Myacyne n'est excrété dans le lait humain, mais il a été démontré qu'il l'était dans le lait de vache après une seule injection intramusculaire. D'autres aminosides ont été excrétés dans le lait maternel. En raison du risque de réactions irréversibles graves de la aminosides chez le nourrisson, il faut décider soit d'interrompre l'allaitement soit d'arrêter le médicament, en tenant compte de l'importance du médicament pour la mère.
Utilisation Pédiatrique
L'innocuité et l'efficacité du sulfate de Kysto-Myacyne N orale chez les patients de moins de 18 ans n'ont pas été établies. Si le traitement d'un patient de moins de 18 ans est nécessaire, Cysto-Myacyne N doit être utilisé avec prudence et la période de traitement ne doit pas dépasser deux semaines en raison de l'absorption par le tractus gastro-intestinal.

Cependant, une administration prolongée pourrait entraîner des concentrations systématiques suffisantes de médicaments pour produire une neurotoxicité, une ototoxicité et/ou une néphrotoxicité.
L'hémodialyse éliminera le sulfate de Cysto-Myacyne N du sang.
En raison de la faible absorption, il est peu probable qu'un surdosage aigu se produise avec Cysto-Myacyne N. cependant, une administration prolongée pourrait entraîner des niveaux de médicaments systémiques suffisants pour produire une neurotoxicité, une ototoxicité et/ou une néphrotoxicité.
L'hémodialyse éliminera la Cysto-Myacyne N du sang.
