What is Terlipressin?
Terlipressin is a synthetic analogue of vasopressin, a naturally occurring hormone in the body also known as antidiuretic hormone. It is a medication primarily used in hospital settings to treat specific complications arising from advanced liver disease and associated circulatory issues.
Mechanism of Action
Terlipressin works by acting as a vasoconstrictor, which means it causes the smooth muscles in the walls of certain blood vessels to narrow. It specifically targets the blood vessels in the abdominal area, known as the splanchnic circulation.
In patients with severe liver scarring, or cirrhosis, blood flow often becomes imbalanced. By narrowing the vessels in the abdomen, terlipressin helps to:
- Reduce pressure in the portal vein: This is the main vein that carries blood from the digestive organs to the liver.
- Redistribute blood flow: By restricting blood flow in the abdomen, the body can redirect more blood toward the kidneys and other vital organs.
- Improve kidney function: In certain conditions where liver failure affects the kidneys, improving blood pressure and flow can help the kidneys filter waste more effectively.
Clinical Applications
Terlipressin is utilized in acute medical situations where rapid stabilization of the circulatory system is required. Its primary applications include:
- Hepatorenal Syndrome (HRS): A type of progressive kidney failure seen in individuals with severe liver damage. Terlipressin helps by improving blood flow to the kidneys.
- Esophageal Varices: It is used to manage bleeding from enlarged veins in the esophagus, a common complication of portal hypertension. By lowering the pressure in these veins, the medication helps to control or prevent active bleeding.
