Evidence for use in Type 2 Diabetes Mellitus (T2DM)
A significant amount of research, primarily using Randomized Controlled Trials (RCTs), was conducted to explore how this treatment was associated with changes in blood sugar measurements in adults with T2DM. This research examined key outcomes such as average blood sugar levels (measured by HbA1c) and fasting plasma glucose. Studies also monitored changes in body weight and the occurrence of low blood sugar events (hypoglycemia).
Findings describe patterns observed in the studies where blood sugar measurements were frequently reported as numerically lower at the end of the study period compared to the beginning or compared to control treatments. Some trials described patterns such as body weight measurements also being lower. However, long-term effects for the persistence of these reported measurements beyond intermediate durations (typically two years) are not fully established.
Evidence for use in Cardiovascular Risk Reduction in T2DM
Dedicated, large-scale Cardiovascular Outcomes Trials (CVOTs) were evaluated in specific T2DM populations to explore outcomes related to serious heart problems. The main research examined the time to occurrence of a composite outcome known as Major Adverse Cardiovascular Events (MACE), which includes measurements of nonfatal heart attack, nonfatal stroke, and cardiovascular death. The research also monitored measurements of all-cause mortality and heart failure hospitalization.
The data show patterns related to MACE events, which was observed in some studies to be numerically lower in the treated group over a long follow-up period (about 3^1/2 years). Changes related to cardiovascular death and hospitalization for heart failure were also described in some studies. The generalizability of these reported patterns to T2DM populations without established cardiovascular disease is not fully established.
Evidence for use in Weight Management (Obesity and Overweight)
For the indication of weight management, a series of RCTs was studied for measured outcomes related to body weight. These studies focused on people with obesity or those with overweight who also had at least one weight-related medical condition. Research examined the percentage change in body weight and also used patient-reported outcomes describing perceived discomfort and physical function.
Studies frequently reported measurements of body weight that were numerically lower at the end of the intermediate-term follow-up (up to 68 weeks) compared to baseline. The research highlights changes measured in which a greater proportion of participants in the treated group achieved prespecified levels of weight change. There is limited information for characterizing the maintenance of the reported weight measurements beyond two years.
What is Still Uncertain About Semaglutide
What is still uncertain includes the precise long-term effects that have been studied beyond three to five years for all outcomes across all indications. The research primarily focused on large groups of adults defined by study inclusion criteria. Data for certain groups, such as the frail elderly or those with rare comorbidities, remain insufficient, and the level of certainty in these groups remains low.
