The recommended initial dose is 20 mg four times a day. After one week treatment with the initial dose, the dose may be increased to 40 mg four times a day unless side effects limit dosage escalation. If efficacy is not achieved within 2 weeks or side effects require doses below 80 mg per day, the drug should be discontinued. Documented safety data are not available for doses above 80 mg daily for periods longer than 2 weeks.

This medication may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert.
Drinking alcohol can increase certain side effects of Dicyclomine (Cyclopam TAB).
Avoid becoming overheated or dehydrated during exercise and in hot weather. Dicyclomine (Cyclopam TAB) can decrease your sweating, which can lead to heat stroke in a hot environment.
Stop using Dicyclomine (Cyclopam TAB) and call your doctor right away if you have serious side effects such as confusion, hallucinations, unusual thoughts or behavior, fast or uneven heart rate, or if you urinate less than usual or not at all.
There are many other medicines that can interact with Dicyclomine (Cyclopam TAB). Tell your doctor about all the prescription and over-the-counter medications you use. This includes vitamins, minerals, herbal products, and drugs prescribed by other doctors. Do not start using a new medication without telling your doctor. Paracetamol (Cyclopam TAB) should not be used in patients who have previously exhibited hypersensitivity to aspirin and/or nonsteroidal antiinflammatory agents. Paracetamol (Cyclopam TAB) should not be given to patients with a recent history of gastrointestinal bleeding or in patients with bleeding disorders (e.g., hemophilia).
Use Dicyclomine (Cyclopam TAB) as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Dicyclomine (Cyclopam TAB) is usually given as an injection at your doctors office, hospital, or clinic. If you will be using Dicyclomine (Cyclopam TAB) at home, a health care provider will teach you how to use it. Be sure you understand how to use Dicyclomine (Cyclopam TAB). Follow the procedures you are taught when you use a dose. Contact your health care provider if you have any questions.
- Dicyclomine (Cyclopam TAB) is for intramuscular (IM) injection only. It is NOT for intravenous (IV) use.
- Do not use Dicyclomine (Cyclopam TAB) if it contains particles, is cloudy or discolored, or if the vial is cracked or damaged.
- Keep this product, as well as syringes and needles, out of the reach of children and pets. Do not reuse needles, syringes, or other materials. Ask your health care provider how to dispose of these materials after use. Follow all local rules for disposal.
- The injection form of Dicyclomine (Cyclopam TAB) is only given short-term when the oral form cannot be taken. Your health care provider will prescribe the oral form of Dicyclomine (Cyclopam TAB) as soon as you are able to take it.
- Use Dicyclomine (Cyclopam TAB) on a regular schedule around the clock, unless your doctor tells you otherwise.
- If you miss a dose of Dicyclomine (Cyclopam TAB), take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.
Ask your health care provider any questions you may have about how to use Dicyclomine (Cyclopam TAB).
Use Paracetamol (Cyclopam TAB) exactly as directed on the label, or as prescribed by your doctor.
Do not use more of this medication than is recommended. An overdose of Paracetamol (Cyclopam TAB) can cause serious harm. The maximum amount for adults is 1 gram (1000 mg) per dose and 4 grams (4000 mg) per day. Using more Paracetamol (Cyclopam TAB) could cause damage to your liver. If you drink more than three alcoholic beverages per day, talk to your doctor before taking Paracetamol (Cyclopam TAB) and never use more than 2 grams (2000 mg) per day. If you are treating a child, use a pediatric form of Paracetamol (Cyclopam TAB). Carefully follow the dosing directions on the medicine label. Do not give the medication to a child younger than 2 years old without the advice of a doctor.
Measure the liquid form of Paracetamol (Cyclopam TAB) with a special dose-measuring spoon or cup, not a regular table spoon. If you do not have a dose-measuring device, ask your pharmacist for one. You may need to shake the liquid before each use. Follow the directions on the medicine label.
The Paracetamol (Cyclopam TAB) chewable tablet must be chewed thoroughly before you swallow it.
Make sure your hands are dry when handling the Paracetamol (Cyclopam TAB) disintegrating tablet. Place the tablet on your tongue. It will begin to dissolve right away. Do not swallow the tablet whole. Allow it to dissolve in your mouth without chewing.
To use the Paracetamol (Cyclopam TAB) effervescent granules, dissolve one packet of the granules in at least 4 ounces of water. Stir this mixture and drink all of it right away. To make sure you get the entire dose, add a little more water to the same glass, swirl gently and drink right away.
Do not take a Paracetamol (Cyclopam TAB) rectal suppository by mouth. It is for use only in your rectum. Wash your hands before and after inserting the suppository.
Try to empty your bowel and bladder just before using the Paracetamol (Cyclopam TAB) suppository. Remove the outer wrapper from the suppository before inserting it. Avoid handling the suppository too long or it will melt in your hands.
For best results from the suppository, lie down and insert the suppository pointed tip first into the rectum. Hold in the suppository for a few minutes. It will melt quickly once inserted and you should feel little or no discomfort while holding it in. Avoid using the bathroom just after inserting the suppository.
Stop using Paracetamol (Cyclopam TAB) and call your doctor if:

you still have a fever after 3 days of use;
-
you still have pain after 7 days of use (or 5 days if treating a child);
-
you have a skin rash, ongoing headache, or any redness or swelling; or
-
if your symptoms get worse, or if you have any new symptoms.
Urine glucose tests may produce false results while you are taking Paracetamol (Cyclopam TAB). Talk to your doctor if you are diabetic and you notice changes in your glucose levels during treatment.
Store Paracetamol (Cyclopam TAB) at room temperature away from heat and moisture. The rectal suppositories can be stored at room temperature or in the refrigerator.

How to use Paracetamol (Cyclopam TAB)
Take this product by mouth as directed. Follow all directions on the product package. If you are uncertain about any of the information, consult your doctor or pharmacist.
There are many brands and forms of acetaminophen available. Read the dosing instructions carefully for each product because the amount of acetaminophen may be different between products. Do not take more acetaminophen than recommended.
If you are giving acetaminophen to a child, be sure you use a product that is meant for children. Use your child's weight to find the right dose on the product package. If you don't know your child's weight, you can use their age.
For suspensions, shake the medication well before each dose. Some liquids do not need to be shaken before use. Follow all directions on the product package. Measure the liquid medication with the provided dose-measuring spoon/dropper/syringe to make sure you have the correct dose. Do not use a household spoon.
For rapidly-dissolving tablets, chew or allow to dissolve on the tongue, then swallow with or without water. For chewable tablets, chew thoroughly before swallowing.
Do not crush or chew extended-release tablets. Doing so can release all of the drug at once, increasing the risk of side effects. Also, do not split the tablets unless they have a score line and your doctor or pharmacist tells you to do so. Swallow the whole or split tablet without crushing or chewing.
For effervescent tablets, dissolve the dose in the recommended amount of water, then drink.
Pain medications work best if they are used as the first signs of pain occur. If you wait until the symptoms have worsened, the medication may not work as well.
Do not take this medication for fever for more than 3 days unless directed by your doctor. For adults, do not take this product for pain for more than 10 days (5 days in children) unless directed by your doctor. If the child has a sore throat (especially with high fever, headache, or nausea/vomiting), consult the doctor promptly.
Tell your doctor if your condition persists or worsens or if you develop new symptoms. If you think you may have a serious medical problem, get medical help right away.

Antiglaucoma Agents
Anticholinergics antagonize the effects of antiglaucoma agents. Anticholinergic drugs in the presence of increased intraocular pressure may be hazardous when taken concurrently with agents such as corticosteroids. Use of Dicyclomine (Cyclopam TAB) in patients with glaucoma is not recommended.
Other Drugs With Anticholinergic Activity
The following agents may increase certain actions or side effects of anticholinergic drugs including Dicyclomine (Cyclopam TAB): amantadine, antiarrhythmic agents of Class I (e.g., quinidine), antihistamines, antipsychotic agents (e.g., phenothiazines), benzodiazepines, MAO inhibitors, narcotic analgesics (e.g., meperidine), nitrates and nitrites, sympathomimetic agents, tricyclic antidepressants, and other drugs having anticholinergic activity.
Other Gastrointestinal Motility Drugs
Interaction with other gastrointestinal motility drugs may antagonize the effects of drugs that alter gastrointestinal motility, such as metoclopramide.
Effect Of Antacids
Because antacids may interfere with the absorption of anticholinergic agents including Dicyclomine (Cyclopam TAB), simultaneous use of these drugs should be avoided.
Effect On Absorption Of Other Drugs
Anticholinergic agents may affect gastrointestinal absorption of various drugs by affecting gastrointestinal motility, such as slowly dissolving dosage forms of digoxin; increased serum digoxin concentration may result.
Effect On Gastric Acid Secretion
The inhibiting effects of anticholinergic drugs on gastric hydrochloric acid secretion are antagonized by agents used to treat achlorhydria and those used to test gastric secretion.

When applied simultaneously with carbamazepine, phenytoin, phenobarbital, primidonom decreases the effectiveness of Paracetamol (Cyclopam TAB), which is caused by an increase in its metabolism and excretion from the body. In applying cholestyramine a period of less than 1 h after administration of Paracetamol (Cyclopam TAB) may decrease of its absorption.
At simultaneous application with lamotrigine moderately increased excretion of lamotrigine from the body.
With the simultaneous use of metoclopramide may increase absorption of Paracetamol (Cyclopam TAB) and its increased concentration in blood plasma.
When applied simultaneously with probenecid may decrease clearance of Paracetamol (Cyclopam TAB), with rifampicin, sulfinpyrazone - may increase clearance of Paracetamol (Cyclopam TAB) due to increasing its metabolism in the liver.
At simultaneous application of Paracetamol (Cyclopam TAB) Guardian with ethinylestradiol increases absorption of Paracetamol (Cyclopam TAB) from the gut.
Enhances the effect of indirect anticoagulants (coumarin derivatives and indandione). Antipyretic and analgesic activity of caffeine increases, reduce - rifampicin, phenobarbital and alcohol (accelerated biotransformation, inducing microsomal liver enzymes).

The pattern of adverse effects seen with Dicyclomine (Cyclopam TAB) is mostly related to its pharmacological actions at muscarinic receptors. They are a consequence of the inhibitory effect on muscarinic receptors within the autonomic nervous system. These effects are dose-related and are usually reversible when treatment is discontinued.
The most serious adverse reactions reported with Dicyclomine (Cyclopam TAB) hydrochloride include cardiovascular and central nervous system symptoms.
Clinical Trials Experience
Because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials of a drug cannot be directly compared to rates in the clinical trials of another drug and may not reflect the rates observed in practice.
The data described below reflect exposure in controlled clinical trials involving over 100 patients treated for functional bowel/irritable bowel syndrome with Dicyclomine (Cyclopam TAB) hydrochloride at initial doses of 160 mg daily (40 mg four times a day)
In these trials most of the side effects were typically anticholinergic in nature and were reported by 61% of the patients. Table 1 presents adverse reactions (MedDRA 13.0 preferred terms) by decreasing order of frequency in a side-by-side comparison with placebo.

Nine percent (9%) of patients were discontinued from Dicyclomine (Cyclopam TAB) because of one or more of these side effects (compared with 2% in the placebo group). In 41% of the patients with side effects, side effects disappeared or were tolerated at the 160 mg daily dose without reduction. A dose reduction from 160 mg daily to an average daily dose of 90 mg was required in 46% of the patients with side effects who then continued to experience a favorable clinical response; their side effects either disappeared or were tolerated.
Postmarketing Experience
The following adverse reactions, presented by system organ class in alphabetical order, have been identified during post approval use of Dicyclomine (Cyclopam TAB). Because these reactions are reported voluntarily from a population of uncertain size, it is not always possible to reliably estimate their frequency or establish a causal relationship to drug exposure.
- Cardiac disorders: palpitations, tachyarrhythmias
- Eye disorders: cycloplegia, mydriasis, vision blurred
- Gastrointestinal disorders: abdominal distension, abdominal pain, constipation, dry mouth, dyspepsia, nausea, vomiting
- General disorders and administration site conditions: fatigue, malaise
- Immune System Disorders: drug hypersensitivity including face edema, angioedema, anaphylactic shock
- Nervous system disorders: dizziness, headache, somnolence, syncope
- Psychiatric disorders: As with the other anti-cholinergic drugs, cases of delirium or symptoms of delirium such as amnesia (or transient global amnesia), agitation, confusional state, delusion, disorientation, hallucination (including visual hallucination) as well as mania, mood altered and pseudodementia, have been reported with the use of Dicyclomine (Cyclopam TAB). Nervousness and insomnia have also been reported.
- Reproductive system and breast disorders: suppressed lactation
- Respiratory, thoracic and mediastinal disorders: dyspnoea, nasal congestion
- Skin and subcutaneous tissue disorder: dermatitis allergic, erythema, rash
Cases of thrombosis, thrombophlebitis and injection site reactions such as local pain, edema, skin color change and even reflex sympathetic dystrophy syndrome have been reported following inadverent IV injection of Dicyclomine (Cyclopam TAB).
Adverse Reactions Reported with Similar Drugs with Anticholinergic/Antispasmodic Action
Gastrointestinal: anorexia,
Central Nervous System: tingling, numbness, dyskinesia, speech disturbance, insomnia
Peripheral Nervous System: With overdosage, a curare-like action may occur (i.e., neuromuscular blockade leading to muscular weakness and possible paralysis).
Ophthalmologic: diplopia, increased ocular tension
Dermatologic/Allergic: urticaria, itching, and other dermal manifestations;
Genitourinary: urinary hesitancy, urinary retention in patients with prostatic hypertrophy
Cardiovascular: hypertension
Respiratory: apnea
Other: decreased sweating, sneezing, throat congestion, impotence. With the injectable form, there may be temporary sensation of light-headedness. Some local irritation and focal coagulation necrosis may occur following the intramuscular injection of Dicyclomine (Cyclopam TAB).

Effects due to the presence of acetylsalicylic acid.

It may also worsen renal function including cases of pre-existing condition (the long-term use can cause chronic kidney disease).

Effects due to the presence of Paracetamol (Cyclopam TAB).
At the doses and duration of recommended treatment, side effects are usually negligible, but the prolonged use of Paracetamol (Cyclopam TAB) in therapeutic doses large (2-4 g per day, or 10-20 tablets Paracetamol (Cyclopam TAB)) may cause the appearance chronic hepatitis. This justifies the recommendations of caution especially in patients with liver disease prior.
Very rarely, it can produce thrombocytopenia.
The acute liver toxicity in overdose massive (doses greater than 8 g of Paracetamol (Cyclopam TAB)) is described in paragraph overdose.
