O cloridrato de fentermina é indicado no tratamento da obesidade exógena como um adjunto de curto prazo (algumas semanas) em um regime de redução de peso baseado em restrição calórica. A utilidade limitada dos agentes desta classe (ver FARMACOLOGIA CLÍNICA) deve ser medido em relação a possíveis fatores de risco inerentes ao seu uso.
Ifa Acxion é indicado como um curto prazo (algumas semanas) adjunto em um regime de redução de peso baseado no exercício, modificação comportamental e restrição calórica no tratamento da obesidade exógena em pacientes com um índice de massa corporal inicial maior ou igual a 30 kg / m² , ou maior ou igual a 27 kg / m² na presença de outros fatores de risco (por exemplo.hipertensão controlada, diabetes, hiperlipidemia).

A utilidade limitada dos agentes dessa classe, incluindo Ifa Acxion, deve ser medida em relação a possíveis fatores de risco inerentes ao seu uso, como os descritos abaixo.

Obesidade Exógena
A dosagem deve ser individualizada para obter uma resposta adequada com a menor dose efetiva.
A dose habitual para adultos é de um comprimido, conforme prescrito pelo médico, administrado pela manhã, com ou sem alimentos. Ifa Acxion não é recomendado em doentes pediátricos com idade igual ou inferior a 16 anos.
A medicação no final da noite deve ser evitada devido à possibilidade de insônia resultante.
Com as mãos secas, remova suavemente o comprimido Ifa Acxion (cloridrato de fentermina ODT) do frasco. Coloque imediatamente o comprimido Ifa Acxion em cima da língua onde ele se dissolverá e engula com ou sem água.
Arteriosclerose avançada, doença cardiovascular sintomática, hipertensão moderada a grave, hipertireoidismo, hipersensibilidade conhecida ou idiossincrasia às aminas simpatomiméticas, glaucoma.
Estados agitados.
Pacientes com histórico de abuso de drogas.
Durante ou dentro de 14 dias após a administração de inibidores da monoamina oxidase (podem ocorrer crises hipertensivas).

WARNINGS
Tolerance to the anorectic effect usually develops within a few weeks. When this occurs, the recommended dose should not be exceeded in an attempt to increase the effect; rather, the drug should be discontinued.
Phentermine hydrochloride may impair the ability of the patient to engage in potentially hazardous activities such as operating machinery or driving a motor vehicle; the patient should therefore be cautioned accordingly.
Drug Dependence: Phentermine hydrochloride is related chemically and pharmacologically to the amphetamines. Amphetamines and related stimulant drugs have been extensively abused, and the possibility of abuse of phentermine hydrochloride should be kept in mind when evaluating the desirability of including a drug as proof of a weight reduction program. Abuse of amphetamines and related drugs may be associated with intense psychological dependence and severe social dysfunction. There are reports of patients who have increased the dosage to many times that recommended. Abrupt cessation following prolonged high dosage administration results in extreme fatigue and mental depression; changes are also noted on the sleep EEG. Manifestations of chronic intoxication with anorectic drugs include severe dermatoses, marked insomnia, irritability, hyperactivity, and personality changes. The most severe manifestation of chronic intoxications is psychosis, often clinically indistinguishable from schizophrenia.
Usage in Pregnancy: Safe use in pregnancy has not been established. Use of phentermine hydrochloride by women who are or who may become pregnant, and those in the first trimester of pregnancy, requires that the potential benefit be weighed against the possible hazard to mother and infant.
Usage in Children: Phentermine hydrochloride is not recommended for use in children under 12 years of age.
PRECAUTIONS
Caution is to be exercised in prescribing phentermine hydrochloride for patients with even mild hypertension.
Insulin requirements in diabetes mellitus may be altered in association with the use of phentermine hydrochloride and the concomitant dietary regimen.
Phentermine hydrochloride may decrease the hypotensive effect of guanethidine.
The least amount feasible should be prescribed or dispensed at one time in order to minimize the possibility of overdosage.

PRECAUTIONS
Coadministration with Other Drug Products for Weight Loss
Ifa Acxion is indicated only as short-term (a few weeks) monotherapy for the management of exogenous obesity. The safety and efficacy of combination therapy with Ifa Acxion and any other drug products for weight loss including prescribed drugs, over-the-counter preparations, and herbal products, or serotonergic agents such as selective serotonin reuptake inhibitors (e.g., fluoxetine, sertraline, fluvoxamine, paroxetine), have not been established. Therefore, coadministration of Ifa Acxion and these drug products is not recommended.
Primary Pulmonary Hypertension
Primary Pulmonary Hypertension (PPH) – a rare, frequently fatal disease of the lungs – has been reported to occur in patients receiving a combination of phentermine with fenfluramine or dexfenfluramine. The possibility of an association between PPH and the use of Ifa Acxion alone cannot be ruled out; there have been rare cases of PPH in patients who reportedly have taken phentermine alone. The initial symptom of PPH is usually dyspnea. Other initial symptoms may include angina pectoris, syncope or lower extremity edema. Patients should be advised to report immediately any deterioration in exercise tolerance. Treatment should be discontinued in patients who develop new, unexplained symptoms of dyspnea, angina pectoris, syncope or lower extremity edema, and patients should be evaluated for the possible presence of pulmonary hypertension.
Valvular Heart Disease
Serious regurgitant cardiac valvular disease, primarily affecting the mitral, aortic and/or tricuspid valves, has been reported in otherwise healthy persons who had taken a combination of phentermine with fenfluramine or dexfenfluramine for weight loss. The possible role of phentermine in the etiology of these valvulopathies has not been established and their course in individuals after the drugs are stopped is not known. The possibility of an association between valvular heart disease and the use of Ifa Acxion alone cannot be ruled out; there have been rare cases of valvular heart disease in patients who reportedly have taken phentermine alone.
Development of Tolerance, Discontinuation in Case of Tolerance
When tolerance to the anorectant effect develops, the recommended dose should not be exceeded in an attempt to increase the effect; rather, the drug should be discontinued.
Effect on the Ability to Engage in Potentially Hazardous Tasks
Ifa Acxion may impair the ability of the patient to engage in potentially hazardous activities such as operating machinery or driving a motor vehicle; the patient should therefore be cautioned accordingly.
Risk of Abuse and Dependence
Ifa Acxion is related chemically and pharmacologically to amphetamine (d- and dll-amphetamine) and to other related stimulant drugs that have been extensively abused. The possibility of abuse of Ifa Acxion should be kept in mind when evaluating the desirability of including a drug as part of a weight reduction program. See Drug Abuse and Dependence and OVERDOSAGE.
The least amount feasible should be prescribed or dispensed at one time in order to minimize the possibility of overdosage.
Usage with Alcohol
Concomitant use of alcohol with Ifa Acxion may result in an adverse drug reaction.
Use in Patients with Hypertension
Use caution in prescribing Ifa Acxion for patients with even mild hypertension (risk of increase in blood pressure).
Use in Patients on Insulin or Oral Hypoglycemic Medications for Diabetes Mellitus
A reduction in insulin or oral hypoglycemic medications in patients with diabetes mellitus may be required.
Risk of Allergic Reactions due to Tartrazine
Ifa Acxion 15 mg and 30 mg ODT contains FD&C Yellow No. 5 (tartrazine), which may cause allergic-type reactions (including bronchial asthma) in certain susceptible individuals. Although the overall incidence of FD&C Yellow No. 5 (tartrazine) sensitivity in the general population is low, it is frequently seen in patients who also have aspirin hypersensitivity.
Nonclinical Toxicology
Carcinogenesis, Mutagenesis, Impairment of Fertility
Studies have not been performed with Ifa Acxion to determine the potential for carcinogenesis, mutagenesis or impairment of fertility.
Use In Specific Populations
Pregnancy
Pregnancy Category X
Ifa Acxion is contraindicated during pregnancy because weight loss offers no potential benefit to a pregnant woman and may result in fetal harm. A minimum weight gain, and no weight loss, is currently recommended for all pregnant women, including those who are already overweight or obese, due to obligatory weight gain that occurs in maternal tissues during pregnancy. Phentermine has pharmacologic activity similar to amphetamine (d- and dllamphetamine). Animal reproduction studies have not been conducted with phentermine. If this drug is used during pregnancy, or if the patient becomes pregnant while taking this drug, the patient should be apprised of the potential hazard to a fetus.
Nursing Mothers
It is not known if Ifa Acxion is excreted in human milk; however, other amphetamines are present in human milk. Because of the potential for serious adverse reactions in nursing infants, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
Pediatric Use
Safety and effectiveness in pediatric patients have not been established. Because pediatric obesity is a chronic condition requiring long-term treatment, the use of this product, approved for short-term therapy, is not recommended.
Geriatric Use
In general, dose selection for an elderly patient should be cautious, usually starting at the low end of the dosing range, reflecting the greater frequency of decreased hepatic, renal, or cardiac function, and of concomitant disease or other drug therapy.
This drug is known to be substantially excreted by the kidney, and the risk of toxic reactions to this drug may be greater in patients with impaired renal function. Because elderly patients are more likely to have decreased renal function, care should be taken in dose selection, and it may be useful to monitor renal function.
Renal Impairment
Ifa Acxion was not studied in patients with renal impairment. Based on the reported excretion of phentermine in urine, exposure increases can be expected in patients with renal impairment. Use caution when administering Ifa Acxion to patients with renal impairment.
Cardiovascular: Palpitações, taquicardia, elevação da pressão arterial.

A experiência com hemodiálise ou diálise peritoneal é inadequada para permitir recomendações a esse respeito. A acidificação da urina aumenta a excreção de fentermina. A fentolamina intravenosa (REGITINA) foi sugerida para possível hipertensão aguda e grave, se isso complica a superdosagem de fentermina.
A menor quantidade possível deve ser prescrita ou dispensada ao mesmo tempo, a fim de minimizar a possibilidade de superdosagem.
Sobredosagem aguda
Manifestações de superdosagem aguda incluem inquietação, tremor, hiperreflexia, respiração rápida, confusão, agressividade, alucinações e estados de pânico. Fadiga e depressão geralmente seguem a estimulação central. Os efeitos cardiovasculares incluem taquicardia, arritmia, hipertensão ou hipotensão e colapso circulatório. Os sintomas gastrointestinais incluem náusea, vômito, diarréia e cãibras abdominais. A sobredosagem de compostos farmacologicamente semelhantes resultou em envenenamento fatal geralmente termina em convulsões e coma.

Intoxicação crônica
Manifestações de intoxicação crônica com drogas anoréticas incluem dermatoses graves, insônia acentuada, irritabilidade, hiperatividade e alterações de personalidade. A manifestação mais grave de intoxicações crônicas é a psicose, geralmente clinicamente indistinguível da esquizofrenia. Vejo Abuso e Dependência de Drogas.
As ações típicas das anfetaminas incluem estimulação do sistema nervoso central e elevação da pressão arterial. Taquifilaxia e tolerância foram demonstradas com todos os medicamentos dessa classe nos quais esses fenômenos foram procurados.
Em termos de taxa e extensão da exposição, os comprimidos desintegrantes por via oral de fentermina são equivalentes às cápsulas e comprimidos de fentermina administrados em condições de jejum.
Após a administração do comprimido de desintegração oral (ODT), a fentermina atinge concentrações máximas (Cmax) após 3,0 a 4,4 horas. A deglutição do ODT após a desintegração com ou sem água não afetou a extensão (AUC) da exposição à fentermina.
A administração do ODT após um café da manhã com alto teor de gordura / alto teor calórico diminuiu a Cmax do phentermine em aproximadamente 5% e a AUC em aproximadamente 12%. Apesar da diminuição da Cmax e da AUC, o ODT de fentermina pode ser administrado com ou sem alimentos.
A ingestão do ODT sem desintegração prévia diminuiu a Cmax da fentermina em aproximadamente 7% e a AUC em aproximadamente 8% em comparação com a ingestão do ODT após a desintegração.
