Kofmowбиган® (solution ophtalmique de maléate de brimonidine / timolol) 0,2% / 0,5% est un agoniste des récepteurs alpha-adrénergiques avec un inhibiteur des récepteurs bêta-adrénergiques indiqué pour la réduction de la pression intraoculaire élevée (IOP) chez les patients atteints de glaucome ou d'hypertension oculaire qui nécessitent un traitement d'appoint ou de remplacement en raison d'une PIO insuffisamment contrôlée; la réduction de la PIO de Комбиган® dosée deux fois par jour était légèrement inférieure à celle observée avec l'administration concomitante de solution ophtalmique de maléate de 0,5% timolol dosé deux fois par jour et de 0,2% de solution ophtalmique de tartrate de brimonidine dosé trois fois par jour.
La dose recommandée est d'une goutte de Комбиган® dans les yeux affectés deux fois par jour, à environ 12 heures d'intervalle. Si plusieurs produits ophtalmiques topiques doivent être utilisés, les différents produits doivent être instillés à au moins 5 minutes d'intervalle.

PRECAUTIONS
Potential For Severe Respiratory Or Cardiac Reactions
Комбиган® contains timolol maleate; and although administered topically can be absorbed systemically. Therefore, the same types of adverse reactions found with systemic administration of beta-adrenergic blocking agents may occur with topical administration. For example, severe respiratory reactions and cardiac reactions including death due to bronchospasm in patients with asthma, and rarely death in association with cardiac failure have been reported following systemic or ophthalmic administration of timolol maleate. Additionally, ophthalmic beta-blockers may impair compensatory tachycardia and increase risk of hypotension.
Cardiac Failure
Sympathetic stimulation may be essential for support of the circulation in individuals with diminished myocardial contractility, and its inhibition by beta-adrenergic receptor blockade may precipitate more severe failure.
In patients without a history of cardiac failure, continued depression of the myocardium with beta-blocking agents over a period of time can, in some cases, lead to cardiac failure. At the first sign or symptom of cardiac failure, Комбиган® should be discontinued.
Obstructive Pulmonary Disease
Patients with chronic obstructive pulmonary disease (e.g., chronic bronchitis, emphysema) of mild or moderate severity, bronchospastic disease, or a history of bronchospastic disease (other than bronchial asthma or a history of bronchial asthma, in which Комбиган® is contraindicated ) should, in general, not receive beta-blocking agents, including Комбиган®.
Potentiation Of Vascular Insufficiency
Комбиган® may potentiate syndromes associated with vascular insufficiency. Комбиган® should be used with caution in patients with depression, cerebral or coronary insufficiency, Raynaud's phenomenon, orthostatic hypotension, or thromboangiitis obliterans.
Increased Reactivity To Allergens
While taking beta-blockers, patients with a history of atopy or a history of severe anaphylactic reactions to a variety of allergens may be more reactive to repeated accidental, diagnostic, or therapeutic challenge with such allergens. Such patients may be unresponsive to the usual doses of epinephrine used to treat anaphylactic reactions.
Potentiation Of Muscle Weakness
Beta-adrenergic blockade has been reported to potentiate muscle weakness consistent with certain myasthenic symptoms (e.g., diplopia, ptosis, and generalized weakness). Timolol has been reported rarely to increase muscle weakness in some patients with myasthenia gravis or myasthenic symptoms.
Masking Of Hypoglycemic Symptoms In Patients With Diabetes Mellitus
Beta-adrenergic blocking agents should be administered with caution in patients subject to spontaneous hypoglycemia or to diabetic patients (especially those with labile diabetes) who are receiving insulin or oral hypoglycemic agents. Beta-adrenergic receptor blocking agents may mask the signs and symptoms of acute hypoglycemia.
Masking Of Thyrotoxicosis
Beta-adrenergic blocking agents may mask certain clinical signs (e.g., tachycardia) of hyperthyroidism. Patients suspected of developing thyrotoxicosis should be managed carefully to avoid abrupt withdrawal of betaadrenergic blocking agents that might precipitate a thyroid storm.
Ocular Hypersensitivity
Ocular hypersensitivity reactions have been reported with brimonidine tartrate ophthalmic solutions 0.2%, with some reported to be associated with an increase in intraocular pressure.
Contamination Of Topical Ophthalmic Products After Use
There have been reports of bacterial keratitis associated with the use of multiple-dose containers of topical ophthalmic products. These containers had been inadvertently contaminated by patients who, in most cases, had a concurrent corneal disease or a disruption of the ocular epithelial surface.
Impairment Of Beta-adrenergically Mediated Reflexes During Surgery
The necessity or desirability of withdrawal of beta-adrenergic blocking agents prior to major surgery is controversial. Beta-adrenergic receptor blockade impairs the ability of the heart to respond to betaadrenergically mediated reflex stimuli. This may augment the risk of general anesthesia in surgical procedures. Some patients receiving beta-adrenergic receptor blocking agents have experienced protracted severe hypotension during anesthesia. Difficulty in restarting and maintaining the heartbeat has also been reported. For these reasons, in patients undergoing elective surgery, some authorities recommend gradual withdrawal of betaadrenergic receptor blocking agents.
If necessary during surgery, the effects of beta-adrenergic blocking agents may be reversed by sufficient doses of adrenergic agonists.
Nonclinical Toxicology
Carcinogenesis, Mutagenesis, Impairment Of Fertility
With brimonidine tartrate, no compound-related carcinogenic effects were observed in either mice or rats following a 21-month and 24-month study, respectively. In these studies, dietary administration of brimonidine tartrate at doses up to 2.5 mg/kg/day in mice and 1 mg/kg/day in rats achieved 150 and 210 times, respectively, the plasma Cmax drug concentration in humans treated with one drop of Комбиган® into both eyes twice daily, the recommended daily human dose.
In a two-year study of timolol maleate administered orally to rats, there was a statistically significant increase in the incidence of adrenal pheochromocytomas in male rats administered 300 mg/kg/day [approximately 25,000 times the maximum recommended human ocular dose of 0.012 mg/kg/day on a mg/kg basis (MRHOD)].
Similar differences were not observed in rats administered oral doses equivalent to approximately 8,300 times the daily dose of Комбиган® in humans.
In a lifetime oral study of timolol maleate in mice, there were statistically significant increases in the incidence of benign and malignant pulmonary tumors, benign uterine polyps and mammary adenocarcinomas in female mice at 500 mg/kg/day, (approximately 42,000 times the MRHOD), but not at 5 or 50 mg/kg/day (approximately 420 to 4,200 times higher, respectively, than the MRHOD). In a subsequent study in female mice, in which post-mortem examinations were limited to the uterus and the lungs, a statistically significant increase in the incidence of pulmonary tumors was again observed at 500 mg/kg/day.
The increased occurrence of mammary adenocarcinomas was associated with elevations in serum prolactin which occurred in female mice administered oral timolol at 500 mg/kg/day, but not at doses of 5 or 50 mg/kg/day. An increased incidence of mammary adenocarcinomas in rodents has been associated with administration of several other therapeutic agents that elevate serum prolactin, but no correlation between serum prolactin levels and mammary tumors has been established in humans. Furthermore, in adult human female subjects who received oral dosages of up to 60 mg of timolol maleate (the maximum recommended human oral dosage), there were no clinically meaningful changes in serum prolactin.
Brimonidine tartrate was not mutagenic or clastogenic in a series of in vitro and in vivo studies including the Ames bacterial reversion test, chromosomal aberration assay in Chinese Hamster Ovary (CHO) cells, and three in vivo studies in CD-1 mice: a host-mediated assay, cytogenetic study, and dominant lethal assay.
Timolol maleate was devoid of mutagenic potential when tested in vivo (mouse) in the micronucleus test and cytogenetic assay (doses up to 800 mg/kg) and in vitro in a neoplastic cell transformation assay (up to 100 mcg/mL). In Ames tests the highest concentrations of timolol employed, 5,000 or 10,000 mcg/plate, were associated with statistically significant elevations of revertants observed with tester strain TA100 (in seven replicate assays), but not in the remaining three strains. In the assays with tester strain TA100, no consistent dose response relationship was observed, and the ratio of test to control revertants did not reach 2. A ratio of 2 is usually considered the criterion for a positive Ames test.
Reproduction and fertility studies in rats with timolol maleate and in rats with brimonidine tartrate demonstrated no adverse effect on male or female fertility at doses up to approximately 100 times the systemic exposure following the maximum recommended human ophthalmic dose of Комбиган®.
Use In Specific Populations
Pregnancy
Teratogenicity studies have been performed in animals. Brimonidine tartrate was not teratogenic when given orally during gestation days 6 through 15 in rats and days 6 through 18 in rabbits. The highest doses of brimonidine tartrate in rats (2.5 mg/kg/day) and rabbits (5 mg/kg/day) achieved AUC exposure values 580 and 37-fold higher, respectively, than similar values estimated in humans treated with Комбиган®, 1 drop in both eyes twice daily.
Teratogenicity studies with timolol in mice, rats, and rabbits at oral doses up to 50 mg/kg/day [4,200 times the maximum recommended human ocular dose of 0.012 mg/kg/day on a mg/kg basis (MRHOD)] demonstrated no evidence of fetal malformations. Although delayed fetal ossification was observed at this dose in rats, there were no adverse effects on postnatal development of offspring. Doses of 1,000 mg/kg/day (83,000 times the MRHOD) were maternotoxic in mice and resulted in an increased number of fetal resorptions. Increased fetal resorptions were also seen in rabbits at doses 8,300 times the MRHOD without apparent maternotoxicity.
There are no adequate and well-controlled studies in pregnant women; however, in animal studies, brimonidine crossed the placenta and entered into the fetal circulation to a limited extent. Because animal reproduction studies are not always predictive of human response, Комбиган® should be used during pregnancy only if the potential benefit to the mother justifies the potential risk to the fetus.
Nursing Mothers
Timolol has been detected in human milk following oral and ophthalmic drug administration. It is not known whether brimonidine tartrate is excreted in human milk, although in animal studies, brimonidine tartrate has been shown to be excreted in breast milk. Because of the potential for serious adverse reactions from Комбиган® in nursing infants, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
Pediatric Use
Комбиган® is contraindicated in children under the age of 2 years. During post-marketing surveillance, apnea, bradycardia, coma, hypotension, hypothermia, hypotonia, lethargy, pallor, respiratory depression, and somnolence have been reported in infants receiving brimonidine. The safety and effectiveness of brimonidine tartrate and timolol maleate have not been studied in children below the age of 2 years.
The safety and effectiveness of Комбиган® have been established in the age groups 2 – 16 years of age. Use of Комбиган® in these age groups is supported by evidence from adequate and well-controlled studies of Комбиган® in adults with additional data from a study of the concomitant use of brimonidine tartrate ophthalmic solution 0.2% and timolol maleate ophthalmic solution in pediatric glaucoma patients (ages 2 to 7 years). In this study, brimonidine tartrate ophthalmic solution 0.2% was dosed three times a day as adjunctive therapy to beta-blockers. The most commonly observed adverse reactions were somnolence (50%-83% in patients 2 to 6 years) and decreased alertness. In pediatric patients 7 years of age or older ( > 20 kg), somnolence appears to occur less frequently (25%). Approximately 16% of patients on brimonidine tartrate ophthalmic solution discontinued from the study due to somnolence.
Geriatric Use
No overall differences in safety or effectiveness have been observed between elderly and other adult patients.

Les effets indésirables suivants ont été rapportés chez 1% à 5% des patients: asthénie, blépharite, érosion cornéenne, dépression, épiphore, écoulement oculaire, sécheresse oculaire, irritation oculaire, douleur oculaire, œdème des paupières, érythème des paupières, sensation de corps étranger , maux de tête, hypertension, sécheresse buccale, somnolence, kératite superficielle,.
Les autres effets indésirables signalés avec les composants individuels sont répertoriés ci-dessous.
Tartrate de brimonidine (0,1% -0,2%)
Goût anormal, réaction allergique, blépharoconjonctivite, vision floue, bronchite, cataracte, blanchiment conjonctival, œdème conjonctival, hémorragie conjonctivale, conjonctivite, toux, vertiges, dyspepsie, dyspnée, fatigue, syndrome grippal, conjonctivite folliculaire, trouble gastro-intestinal, hypercholestérolémie, hypotension, infection (principalement les rhumes et les infections respiratoires) hordeolum, insomnie, kératite, couvercle en croûte, trouble du couvercle, douleur musculaire, sécheresse nasale, réaction allergique oculaire, pharyngite, photophobie, éruption cutanée, rhinite, infection des sinus, sinusite, kératopathie ponctuée superficielle, déchirure, symptômes respiratoires supérieurs, défaut du champ visuel, détachement vitreux, trouble vitreux, flotteurs vitreux, et aggravé l'acuité visuelle.
Timolol (administration oculaire)
Corps dans son ensemble : douleur thoracique; Cardiovasculaire: Arythmie, bradycardie, arrêt cardiaque, insuffisance cardiaque, ischémie cérébrale, accident vasculaire cérébral, claudication, mains et pieds froids, œdème, bloc cardiaque, palpitations, œdème pulmonaire, phénomène de Raynaud, syncope et aggravation de l'angine de poitrine ; Digestif: anorexie, diarrhée, nausées; Immunologique: Lupus érythémateux disséminé ; Système nerveux / psychiatrique: Augmentation des signes et symptômes de myasthénie grave, insomnie, cauchemars, paresthésie, changements de comportement et troubles psychiques, y compris confusion, hallucinations, anxiété, désorientation, nervosité et perte de mémoire; Peau: Alopécie, éruption psoriasiforme ou exacerbation du psoriasis ; Hypersensibilité: Signes et symptômes de réactions allergiques systémiques, y compris anaphylaxie, œdème de Quincke, urticaire et éruption cutanée généralisée et localisée ; Respiratoire: Bronchospasme (principalement chez les patients atteints d'une maladie bronchospastique préexistante), dyspnée, congestion nasale, insuffisance respiratoire, infections des voies respiratoires supérieures ; Endocrinien: Symptômes masqués d'hypoglycémie chez les patients diabétiques; Sens spéciaux: diplopie, détachement choroïdien après chirurgie de filtration, œdème maculaire cystoïde, diminution de la sensibilité cornéenne, pseudopémphigoïde, ptose, changements réfractifs, acouphènes; Urogénital: diminution de la libido, impuissance, maladie de Peyronie, fibrose rétropéritonéale.
Expérience post-commercialisation
Les réactions suivantes ont été identifiées lors de l'utilisation post-commercialisation de solutions ophtalmiques de tartrate de brimonidine, de solutions ophtalmiques de timolol, ou les deux en combinaison, en pratique clinique. Parce qu'ils sont signalés volontairement par une population de taille inconnue, des estimations de fréquence ne peuvent pas être faites. Les réactions, qui ont été choisis pour l'inclusion en raison de leur sérieux, fréquence des rapports, connexion causale possible aux solutions ophtalmiques de tartrate de brimonidine, solutions ophtalmiques timolol, ou une combinaison de ces facteurs, inclure: érythème des paupières s'étendant jusqu'à la joue ou au front, hypersensibilité, iritis, kératoconjonctivite sicca, la myosis, nausée, réactions cutanées (y compris l'érythème, éruption cutanée, et vasodilatation) et la tachycardie. Chez les nourrissons, une apnée, une bradycardie, un coma, une hypothermie, une hypotonie, une léthargie, une pâleur, une dépression respiratoire et une somnolence ont été rapportés.
Bloqueurs oraux de timolol / bêta oral
Les effets indésirables supplémentaires suivants ont été rapportés lors de l'expérience clinique avec le maléate de timolol ORAL ou d'autres agents bêta-bloquants ORAL et peuvent être considérés comme des effets potentiels du maléate de timolol ophtalmique: Allergique: Éruption érythémateuse, fièvre associée à des douleurs et des maux de gorge, laryngospasme avec détresse respiratoire ; Corps dans son ensemble : Diminution de la tolérance à l'exercice, douleur aux extrémités, perte de poids ; Cardiovasculaire: Vasodilatation, aggravation de l'insuffisance artérielle ; Digestif: Douleurs gastro-intestinales, hépatomégalie, colite ischémique, thrombose artérielle mésentérique, vomissements ; Hématologique: Agranulocytose, purpura non thrombocytopénique, purpura thrombocytopénique ; Endocrinien: Hyperglycémie, hypoglycémie ; Peau: Pigmentation accrue, prurit, irritation cutanée, transpiration ; Musculo-squelettique: Arthralgie; Système nerveux / psychiatrique: Un syndrome réversible aigu caractérisé par une désorientation du temps et du lieu, une diminution des performances en neuropsychométrie, une diminution de la concentration, une labilité émotionnelle, une faiblesse locale, une dépression mentale réversible progressant vers la catatonie, un sensorium légèrement trouble, un vertige ; Respiratoire: Obstruction bronchique, rales ; Urogénital: Difficultés d'urination.
Des cas de surdosage par inadvertance avec une solution ophtalmique de timolol ont entraîné des effets systémiques similaires à ceux observés avec les agents de blocage bêta-adrénergiques systémiques tels que des étourdissements, des maux de tête, un essoufflement, une bradycardie, un bronchospasme et un arrêt cardiaque. À l'exception de l'hypotension, il existe des informations très limitées sur l'ingestion accidentelle de brimonidine chez l'adulte. Des symptômes de surdosage de brimonidine ont été rapportés chez des nouveau-nés, des nourrissons et des enfants recevant des solutions ophtalmiques à la brimonidine dans le cadre du traitement médical du glaucome congénital ou par ingestion orale accidentelle. Le traitement d'une surdose orale comprend une thérapie de soutien et symptomatique; une voie aérienne brevetée doit être maintenue.
Absorption
L'absorption systémique de la brimonidine et du timolol a été évaluée chez des volontaires sains et des patients après administration topique de Комбиган®. Les volontaires normaux dosés avec une goutte de Комбиган® deux fois par jour dans les deux yeux pendant sept jours ont montré des concentrations plasmatiques maximales de brimonidine et de timolol de 30 pg / ml et 400 pg / ml, respectivement. Les concentrations plasmatiques de brimonidine ont culminé 1 à 4 heures après l'administration oculaire. Les concentrations plasmatiques maximales de timolol se sont produites environ 1 à 3 heures après l'administration.
Dans une étude croisée de Комбиган®, tartrate de brimonidine 0,2%, et timolol 0,5% administré deux fois par jour pendant 7 jours chez des volontaires sains, la courbe moyenne de la zone de brimonidine sous le plasma-concentration-temps (AUC) pour Комбиган® était de 128 ± 61 pg • h / mL contre 141 ± 106 pg • h / mL pour les traitements de monothérapie respectifs; les valeurs moyennes de Cmax de la brimonidine étaient comparables après le traitement par Комбиган® par rapport à la monothérapie (32,7 ± 15 pg / ml contre 34,7 ± 22,6 pg / ml, respectivement). Dans une étude parallèle chez des patients traités deux fois par jour avec Комбиган®, deux fois par jour avec 0,5% de timolol, ou trois fois par jour avec 0,2% de tartrate de brimonidine, les concentrations plasmatiques d'une heure après la dose de timolol et de brimonidine étaient environ 30 à 40% inférieures avec Комбиган® que leurs valeurs respectives de monothérapie. Les concentrations plasmatiques plus faibles de brimonidine avec Комбиган® semblent être dues à un dosage deux fois par jour pour Комбиган® contre un dosage trois fois avec du tartrate de brimonidine 0,2%.
Distribution
La liaison protéique du timolol est d'environ 60%. La liaison protéique de la brimonidine n'a pas été étudiée.
Métabolisme
Chez l'homme, la brimonidine est largement métabolisée par le foie. Le timolol est partiellement métabolisé par le foie.
Excrétion
Dans l'étude croisée chez des volontaires sains, la concentration plasmatique de brimonidine a diminué avec une demi-vie systémique d'environ 3 heures. La demi-vie systémique apparente du timolol était d'environ 7 heures après l'administration oculaire.
L'excrétion urinaire est la principale voie d'élimination de la brimonidine et de ses métabolites. Environ 87% d'une dose radioactive de brimonidine administrée par voie orale a été éliminée en 120 heures, dont 74% dans l'urine. Le timolol inchangé et ses métabolites sont excrétés par le rein.
